Summer Research Program onboarding materials

For Buffalo area interns:

Below please find the links to the paperwork you will need to read and complete prior to beginning your training as a Summer Research Program intern. Follow the steps below for each item:

1.  Read the documents in Section A. They are policies and other informational materials.

2.  The documents in Section B must be completed, printed, signed, and returned to Educational Affairs prior to starting your training.

3.  In addition to completing the materials in Section B, you must also have health clearance prior to beginning your role as an intern. Please print out the "Health & Fitness Statement" below, have it completed by your health care provider, and returned to us by Friday, May 24.

4.  Please note that you must bring your original work authorization documents as referenced on "Form I-9" (Section B) on the day of your appointment.

5.  If you are less than 18 years old , you must also bring your working papers with you to onboard. If you do not have working papers, contact your high school's guidance office for the application forms.

6.  Once you have completed all the paperwork in Section B, please contact Ms. Cathleen Allen in Educational Affairs to make an appointment to complete the onboarding process. Ms. Allen may be reached at 716-845-4632.

For Out-of-Town interns:

5.  If you are less than 18 years old and a New York State resident, bring your working papers with you to orientation. If you do not have working papers, contact your high school's guidance office for the application forms.

6.  If you are less than 18 years old but not a resident of New York State, we will be contacting your parents directly regarding obtaining working papers for you.

7.  Bring all completed paperwork and associated identification (see numbers 4 and 5 above) to orientation.
